IMPORTANT: All disabled persons are now required  to be recertified by their doctor, using the above form, prior to renewing their parking permit or replacing their lost/stolen parking   permit.

Temporary Disability: A temporary disabled parking permit is available for up to 180 day period at a cost of $15. The disabled person and their doctor are required to sign for the permit using the same form stated above.
